Qualtrics Account Setup (Faculty and Staff Only)

Qualtrics is the online survey tool chosen by UMHB for academic research. All human research conducted at, by, or under the auspices of this institution, whether funded or not and whether conducted by the administrators, faculty, staff, or students, must be reviewed and approved before the research begins – including subject recruitment. Getting Started

The first step to using Qualtrics is creating an account through a one time self-enrollment process. All that is needed is a email address (note - students cannot self-enroll with a email address). Follow the steps below to create your account:

  1. Go to
  2. Click on the "Please click here to create an account" link
  3. Enter your email address and enter a password
  4. Click the Get Started button
  5. Enter your first and last name and click the Finish button
  6. You will receive an email from Qualtrics with a link to confirm your account and login
